Family Health Centers Using Opioid Settlement Funds to Strengthen Recovery and Reach More People

(December 2025) Family Health Centers’ Rise Above Addiction Program is helping expand access to life-saving treatment for individuals affected by opioid use disorder, made possible through funding from the Opioid Settlement Fund administered by Louisville Metro Public Health & Wellness.

Through this investment, Family Health Centers has strengthened its ability to provide medications for opioid use disorder (MOUD), care coordination, and wraparound support for patients seeking recovery. Over the past year, the program has focused on building capacity, improving care coordination, and learning how to best meet the needs of the people it serves.

One important lesson has been recognizing how patient needs differ from early assumptions. Many individuals receiving MOUD services are covered by Medicaid and already have access to medications and transportation, reducing the need for direct financial assistance in those areas. In response, Family Health Centers is shifting remaining resources to targeted outreach, including radio and community messaging, to better reach uninsured, underinsured, and limited English proficiency (LEP) populations who may not yet know help is available.

Historically, Family Health Centers’ medication for opioid use disorder (MOUD) services have been concentrated in West Market (40212) and Phoenix Health Care for the Homeless (40202). With support from opioid settlement funding, Family Health Centers is now expanding services to Iroquois (40215) through the recruitment of a part-time Psychiatric Advanced Practice Registered Nurse (APRN). This provider will be located at the Iroquois Clinic, where an on-site pharmacy was recently installed, creating a fully integrated model of care.

The Psychiatric APRN will focus on MOUD referrals and provide integrated psychiatric care, supporting patients with co-occurring mental health and substance use needs. This expansion will allow Family Health Centers to transition a portion of its current caseload to the new site while also accepting new referrals generated through targeted outreach efforts, including outreach to uninsured, underinsured, and limited English proficiency populations. This grant runs through December 2026, during which time Family Health Centers aims to build on this progress, refine outreach strategies, and continue delivering evidence-based treatment that meets patients where they are. The Rise Above Addiction Program reflects the intent of opioid settlement funding: to support effective, community-driven solutions that save lives and strengthen pathways to recovery.
