Family Health Centers has programs to help make our services affordable for everyone.
Family Health Centers services and prescriptions are available with a sliding-fee discount based on your household size and income.
Family Health Centers accepts all forms of Kentucky Medicaid insurance, and most forms of Medicare and private insurances. Please bring your insurance cards to every appointment.
Family Health Centers can help you apply for free or low cost health insurance. This is a free service, available to anyone in the community.
If you have questions about a bill you received or if you would like to set-up a payment plan, please call 502-795-1772. If you would like to pay your bill online, use the link below.
For patients not using health insurance, Family Health Centers will can provide you a cost estimate of your scheduled services, if the appointment is made more than 3 days in advance. You can request a written cost estimate of services by calling 502-772-8102 or emailing estimate@fhclouisville.org.
