Behavioral Health Services $25.00 a visit
Dental Services $40.00 a visit

I am currently the chairperson for the Kentucky Oral Health Coalition and a member of the DentaQuest Kentucky Dental Advisory Committee. I am also gratis faculty for both the University of Kentucky College of Dentistry and the University of Louisville School of Dentistry. I am a past member of the University of Louisville School of Dentistry Ad Hoc Committee on the Extramural Education Program, past member of the Community Research Advisory Board for University of Louisville Clinical and Translational Sciences Institute, and a past clinical advisor for MPH students at the University of Louisville School of Public Health.
