Request a patient’s medical records, laboratory or radiology report
All requests for a patient’s medical records, lab or radiology reports must be received in writing. Requests can be faxed to the FHC location where the patient usually receives their health care. If the request is immediate, (i.e the patient is present in the provider’s office or hospital) call the FHC location the patient usually receives their care and request to speak with Medical Records.
Radiology film requests must be requested through the FHC Radiology office at 772-8145.
