Back to school already?

School just let out, but before you know it August 15 will be here.  Every year we get a rush of kids needing thier JCPS required immunizations, physicals, and dental exams.  Beat the back to school rush, and get your exams early!  Here is what JCPS requires:

First time students, all 4 year olds, & all 6th graders Immunizations certificate*, a complete shot history for all vaccines since birth. Due within 2 weeks of your child’s attendance.
First time students & all 6th gradersKentucky Preventative Health Care Examination* (a physical). This does not include physicals required for sports.Due within 2 months of your child’s first day of attendance.
First time students, ages 3, 4, 5, or 6 years oldKentucky Vision Examination
This exam must be completed by an optometrist or ophthalmologist.
Due by January 1ST of the first year that your child is enrolled in a public school .(including preschool and Head Start)
First time students, ages 5 or 6 years oldDental Examination* Due by January 1ST of the first year that your child is enrolled in a public school.

*Family Health Centers, Inc. can provide these services for your child. 

Please note that new this year:

  • All JCPS students will be required to have proof of two doses of the Hepatitis A vaccine.
  • All students ages 16 years or older are also required to show proof of having two doses of the Meningococcal ACWY vaccine.

Call your Family Health Centers location to schedule an appointment with their pediatric provider or dentist.
