Beginning March 1, 2024 the minimum fee will increase for the first time in 11 years. The new minimum fees are:

Medical $25.00 a visit
Behavioral Health Services $25.00 a visit
Dental Services $40.00 a visit

Planning Your Visit

Family Health Centers welcomes new patients at our seven locations throughout Louisville Metro. All sites provide primary care to adults and have on-site laboratory services. Most offer women’s health services, pediatrics, and counseling services. All Family Health Centers patients may access all services, though travel to another location is sometimes necessary. Make an Appointment

Making an appointment is easy.  Family Health Centers offers patients’ in-person appointments, video or telephone appointments. Video and telephone appointments are helpful if you are unable to make an in-person visit to one of our sites.  Call (502) 774-8631 to schedule any appointment type.

Video

A video visit is seeing your health care provider on your smart phone or tablet from your home. You will be able to see and talk with your provider about your health concerns, come up with a care plan, and get prescriptions refilled. 

All you need is a smart phone to do a Video Visit.  When you call to schedule an appointment, let us know you want a video visit. Family Health Centers will send you a secure text message at the time of your appointment.

Appointment Reminders

A few things to remember for your appointment:

  • Bring all of your medications with you to every appointment.
  • If you have insurance, bring your insurance card. Family Health Centers accepts most insurances.
  • If you cannot keep an appointment, please call us to cancel or reschedule the appointment.
  • Arrive on time for your appointment, or call to let us know if you are running late.

Transportation

Most locations are on a Transit Authority of River City (TARC) bus line. Parking is available at all Family Health Centers sites. Ask about help getting a bus or taxi voucher.

Language Services

Family Health Centers provides free, trained interpreters for any patient who cannot communicate in English or who is deaf or hard of hearing.

After Hours

If you need to contact a FHC provider when we are closed, call (502) 774-8631. An answering service will help connect you to a provider on-call. The provider on-call will return your call usually within 30 minutes or less. Your health insurance card may also include an after-hours nurse line for your questions.
