Insurance and Payment

FHC accepts several major health and dental insurance plans including: Passport Health Plan, Anthem Kentucky (excluding the HMO), Humana PPO and Tricare (Champus and ChampusVA). FHC also accepts Medicaid, Medicare Part B, and those covered under the Kentucky Children’s Health Insurance Program (KCHIP). For Medicare patients there is no deductible for office visits.

Patients who do not have health insurance are still eligible for services, though a small payment may be requested at time of service. Most major credit/debit cards are also accepted. A Patient Account Specialist is available to help answer any questions you may have, and to discuss payment options if necessary.
