Becoming a Patient
FHC offers a variety of programs and services and always welcomes new patients. Whether you live in one of our neighborhoods, or simply would like to get established with a physician, FHC can provide you with a medical home.
FHC’s wide range of services include: adult and pediatric medicine, obstetrics and gynecology, dentistry, mental health, social services, health education, and after-hours care. Eligible patients may receive fee payment discounts through FHC’s sliding scale program. Each of FHC’s six Health Centers in the Louisville Metro area is on a Transit Authority of River City (TARC) bus line, and a stop is made at each site. FHC can also help with transportation for you to see your medical provider through arrangements with other local carriers.
